Lasik eye surgery – one of the most popular elective procedures performed today – continues to grow in popularity as more and more people choose to leave their eyeglasses and contact lenses in the dust and embrace clearer vision through advanced surgical procedures.

The Lasik procedure is one that had its start in a myriad of technological breakthroughs dating back to the late 1940s but did not come together into the procedure we know today until 1990. The Lasik procedure as we know it today was first performed in the United States in 1991.

Lasik itself stands for Laser-Assisted In Situ Keratomileusis and is a combination of several technologies developed throughout the years. It essence, Lasik is refractive eye surgery wherein corrections are made to the way in which our eyes refract – or bend – light.

In a normally functioning eye, light is received through the lens of the eye – the segment located directly behind the colored part – or iris – of the eye. Once the lens receives light it then refracts or bends to the retina of the eye – found in the grouping of cells lining the back of the eye. From there, the retina converts these light signals into a message that it can deliver to the optic nerve in our brain. The optic nerve receives these signals as a visual image or picture.

Through heredity, illness, injury, or aging our eyes can experience refractive difficulties in the way of nearsightedness, farsightedness, and astigmatism, among other conditions. When this occurs light is not refracted properly and eventually the message or signal that the optic nerve receives is translated into a blurry image.

During the Lasik procedure surgeons use laser technology to resurface the cornea – the transparent covering of the eye – thus clearing the way for crisper, cleaner refraction of light. The Lasik procedure should always begin with the proper choosing of a surgical center through comprehensive research. The surgeon who will perform the procedure will ordinarily begin with one or two pre-operative visits during which time candidacy for the procedure is determined and overall health is assessed.

The Lasik procedure itself begins with the numbing of the eye with the use of eye drops. Once the eye is numbed, the patient is asked to lie on his/her back and the eye itself is held open with a speculum that is placed on the upper and lower eyelids. The eye is then immobilized with the use of a corneal suction ring. Once the eye is immobilized the surgeon makes a very small incision in the top layer of the cornea in order to create a flap.

The flap is then folded back to reveal the mid-section of the cornea – known as the stroma. The surgeon then used low-powered laser technology to resurface the stroma after which the flap is replaced over the eye. The procedure itself takes very little time – usually no more than 10 minutes for each eye.

Healing begins immediately and happens naturally. Most patients report little to no discomfort during the Lasik procedure and only mild discomfort following it – which generally disappears within 24 hours following the surgery.

You’ll see advertisements offering lasik eye surgery for as little as $500 (remember that’s per eye), but that is nothing like the true average cost of lasik eye surgery. One published estimate suggests that in reality, fewer than 3% of treatments cost less than $1000 per eye.

Certainly, prices have been rising steadily since the start of this millennium. One study suggested that the average price in 2002 was around $1600. A year later it had passed $1700; by 2004 it was another $100 higher and by 2005 it was nudging $2000. Prices seem to have stabilized a little since then; one of the major players, TLC Laser Eye Centers, says that in 2006 its prices were still around $2000. Supply and demand certainly influences prices. Market Scope, which is a leading supplier of information in this market, estimates that at the turn of the century there were about 1.44m lasik surgeries in the United States.

Three years later the figure had dropped to 1.15m, but has recovered substantially since then. Part of the reason for the fluctuation was a rash of lawsuits that drove a number of the discount surgeries out of business; those that survived had to move prices up to ensure they can deliver quality procedures and stay in business. It’s difficult even to talk about the “average cost” of lasik eye surgery because prices are anything but standardized, and phrases such as “lasik cost” or “lasik price” are almost impossible to pin down.

However, if you read the fine print carefully, add back all the exclusions, you’ll find that the true average cost of lasik eye surgery is closer to that figure of $2000 per eye than the $500 suggested by some discount lasik clinics. If you live on the West Coast average prices will be higher than elsewhere. If you pick an experienced surgeon (and other research shows that experienced surgeons have a much lower percentage of “problem” procedures) your cost will go up still further. The base cost per eye will increase by between $350 to $500 if you need “custom lasik” treatment such as wavefront technology, which is used to map and measure the eye’s visual field with great accuracy – which in turn results in a higher success rate.

If the surgeon uses IntraLase (a type of laser) to create the LASIK flap rather than a microkeratome (a type of blade) expect the lasik eye surgery price to go up by a further $250 to $500 per eye. The benefit of the different (and more expensive) Intralase technology is that most surgeons using it report a substantially lower incidence of problems with the “flap” that is created as part of the Lasik procedure. The equipment used both in testing and in the actual procedure can vary widely. Eye clinics will analyze the tear film, measure the thickness of the cornea, map the corneal topography, measure the pupil size, use different types of laser – and have good arguments in favour of the processes and equipment they offer you.

So it’s easy to see how the average cost of laser eye surgery edges upwards from what is in effect a base price quoted in the advertisements. Experience, equipment, procedures – all play a part in moving the price along. Remember to read the fine print; there is no substitute for an informed decision about what the full costs of the treatment will be; averages can be very deceptive when dealing with something as variable as the average cost of lasik eye surgery.

Some very well designed and rigorous Australian research has found that to avoid eye health problems as you get older you’ll want to be eating lots of fish, olive oil and nuts, while avoiding those tasty (but troublesome) trans fats.

What these foods all have in common is they contain healthy fats, and these are thought to reduce the risk for age related macular degeneration, known as AMD.

The University of Sydney researchers found those who ate a serving of fish each week were 31% less likely to develop early AMD than those who didn’t eat fish.

A few servings of nuts per week cut the AMD risk by 35%.

A second team from the University of Melbourne found that those who consumed the most omaga-3 fatty acids were at 15% lower risk of the early stage of AMD. Those who ate at least 100 milliliters (7 tablespoons) of olive oil per week were about half as likely to develop late AMD as those who took in less than 1 mL each week.

Another important finding that came out of the Melbourne work was the role of trans fats.

Subjects who ate Trans fats regularly were 1.76 times more likely to develop late AMD during the follow up period of the study.

This isn’t the first study to suggest that omega-3 fatty acids might help to safeguard vision.

Researchers from places as far flung as Iceland and the Netherlands, as well as U.S. scientist Paul SanGiovanni with the National Eye Institute in Maryland seem to agree that omega-3 fatty acids have an impact on AMD. He believes that the healthy fats might help protect against this age related eye disease by fighting inflammation.

When you understand the mechanics of the eye, it makes sense that omega-3 fatty acids would be helpful.

Both nervous tissue and the retina have high levels of these fatty acids, especially a very beneficial one called DHA (docosahexaenoic acid).

Moving beyond the observational work that’s been done to date, San Giovanni has a clinical trial underway where subjects take omega-3 fatty acids, another compound or a placebo. Unfortunately, the results of this work won’t be available for another four years.

If you’re worried about AMD, or you have risk factors, what should you do?

“I recommend to my patients that they consume a diet rich in omega-3 fatty acids and fish, especially salmon, sardines and mackerel, as well as nuts and green, leafy vegetables,” says Johanna Seddon, M.D. ScM, of the New England Eye Center and Tufts University School of Medicine. “Those not able to consume these foods should consider taking supplements containing fish oil or lutein.”

AMD is the top cause of blindness in those 65 and older. The only risk factors experts can point to for sure are a person’s age, genetics, obesity and smoking history.

This disease slowly, almost imperceptibly and without pain destroys sharp central vision, the vision you need to see things clearly, as well as for daily tasks like reading, watching TV, recognizing faces and driving.

There are two forms of AMD, wet and the more common dry and the type determines the treatment used to preserve your sight.

Recent estimates suggest that 1.75 million people have advanced AMD, and another 7.3 million remain in the early stages of the disease. Right now, treatment for AMD is limited to the latter stages.
